Sometimes families need help with challenging transitions, endless decision-making and navigating a complex healthcare system. Most families find it comforting to have someone who is outside the emotional landscape of your family to help by doing research, talking to providers, organizing paperwork, or finding those "needle in a haystack" resources. We are the go-to person you can turn to when things get overwhelming.
No care plan is cookie cutter because your situation is unique. We customize our services for your family. We are your advocates on this often-difficult journey and help you track the endless details.
In our introductory call it’s important that we spend time getting to know each other and we understand your needs before you decide if working together is a good fit. You can ask us any question you.
We can assist family members with decisions by evaluating options, facilitating difficult conversations, providing objective input and help you plan next steps for a loved one. This may include individual or group consultation with family members.
If long term care in a skilled nursing facility is required, we provide guidance and assistance in locating options for you; we will help you understand some of what you need to know about the skilled nursing environment.
This can include in-home care evaluation, referrals to in-home care agencies, initial home safety assessment, and/or mobility and safety equipment procurement. Resources for home modifications can be provided.
We can help you determine the best professional resources, whether it be medical (skilled nursing, rehabilitation, cognitive evaluations) or non-medical (legal, financial, social, community services).
At your request, we will check in on your loved one when you are out of town or unable to visit yourself. Our Advisors are skilled at reading between the lines, noticing subtle changes, and asking the right questions of caregivers and will report back to you.
