They opened a local placement agency and learned everything there was to know about finding the right care situation for an older adult needing help with activities of daily living. They came to the Senior Care space with successful business development skills in their past as well as personal family experience finding care solutions. Souban Houn-Cairns received her Bachelors in Business Management from Arizona State University and Masters in Healthcare Informatics from Grand Canyon University. She completed her master program’s capstone project on Care Transitions, where she studied the unique challenges that present during transfer of care of the elderly, particularly those with complex health issues or have unique mental, physical and social needs.
Souban has over 18 years experience in healthcare consulting with expertise in implementing evidence-based care and best practices. In her previous role as senior consultant at one of the largest healthcare consulting firm in the country, Souban has managed and supported complex project implementations across both large and small healthcare systems. Most recently, Souban designed and implemented healthcare technology solutions to ensure accuracy in documentation and accessibility in patient information to drive better patient care at one of the most prestigious academic institution in New York.
Matt received his Bachelors in Business Management from Nichols College and was Project Director for one of the most successful federally funded Challenge Grants that bridged the gap of the continuum of care across hospitals systems, long-term care, home health and health information exchanges.
As a Healthcare Consulting professional, Matt specialized in managing projects that directly impact acute and post acute care, clinical decision support tools, and collaboration of clinical stakeholders across Health Care Systems. As part of the federally approved grant, Matt was directly responsible for improving post acute care transitions by impacting communications from hospital to post acute facilities. The project focused on introducing technology that captured Assisted Daily Living (ADL) scores that were disseminated through the healthcare community to drive down ED and hospital Admissions. Matt brings extensive, on-the-ground experience in implementation of clinical technologies in the health systems across acute and post acute care.
